ABSTRACT

Background:Maternal mortality is an important indicator of a woman’s health. Even though MMR has reduced dramatically death from hemorrhage remains prominent even in developed countries. Abruptio placentae is an acute obstetrical emergency. Methods: This study was done in the Department of Obstetrics and Gynecology Government Medical College and Rajindra Hospital Patiala to assess clinical and coagulation profile of patients presenting with abruptio placentae and analyze maternal and fetal outcome. Result: A total of 73 patients with abruptio placentae were studied, giving an incidence of 3.09%.Of these 76.7% patients were in the age group of 21-30 years. (60%) were either nulliparous or primiparous. (65.75%) patients belonged to rural area with no regular antenatal checkup. The 90.41% patients belong to lower socio- economic status and 56% were iIIiterate. The mean gestational age in our study was 34.5 weeks and classical presentation of bleeding per vaginum associated with pain was seen in 54.7%. Most common associated risk factor found to be hypertension (58.9%), followed by PROM (13.7%) and trauma (2.74%). Conclusion: The main complications observed were shock, DIC and PPH. There was only one maternal death in our series; Mode of delivery was Vaginal in 53 Cases (72.60%) and LSCS in 20 (27.40%) Perinatal mortality was observed in 40 patients (54.79%).

ABSTRACT

Objective To investigate the significance of expressions of carcinoembryonic antigen(CEA),fibrinogen degrada-tion products(FDP)and D-dimer(DD)in the pleural effusion specimens for differential diagnosis between the benign and malignant pleural effusion.Methods 40 patients with benign pleural effusion patients and 30 patients with malignant pleural effusion were divided into benign or malignant group.The levels of CEA in pleural effusion was detected by electrochemilu-minescence,and FDP and DD were measured by turbidimetry.The value of diagnosis of three separate indicators and combine detection was compared by ROC analysis.Results The levels of CEA,FDP and DD in the pleural effusion specimens of ma-lignanat group were significantly higher than the benign group(t=2.523~3.889,all P<0.01).The sensitivity of combined detection of CEA,FDP and DD was 76.7%,and the rate of correct diagnosis was 82.9%.The sensitivity and diagnostic dffeiciency of combined detection were higher than separate indicators(χ2=1.036~3.324,all P<0.05).Conclusion Com-bined detection of CEA,FDP and DD is helpful to differential diagnosis of benign and malignant pleural effusion.

ABSTRACT

Objective To evaluate the clinical practicality of two different D-dimer assays by studying the correlation of the testing results by two different D-dimer reagents with fibrin degradation products (FDP)result values and analyzing the Clinical alignment.Methods A representative cross-sectional study of 762 clinical patients’plasma specimens were taken to measure the results of D-dimer and FDP by using Sysmex CA-1500 automated coagulation analyzer,D-DIMER detection rea-gent respectively Siemens Innovance D-DIMER and Dade Behring D-DIMER Plus reagents.Then rating frequency analysis and Spearman correlation test were applied to analyze the correlation of measurement results.Results Overall,to the same patient,the correlation of D-dimer tested by Siemens Innovance D-DIMER Reagent with FDP level was significant positive (r= 0.954,P 5 mg/L FEU,the correlation was the best (P5μg/ml)and FDP positive rate in both different cases when D-dimer results was also positive,FDP positive rate in Innovance D-DIMER Reagent group (about 65.76%)was lower than DD Plus Reagent group,it indicated that D-dimer results tested by Innovance D-DIMER Reagent could raise before FDP reacted,so D-dimer had a higher sensitivity than FDP.Conclusion It can provide a more precise clinical diagnosis and treatment infor-mation when using Siemens Innovance D-DIMER Reagent to detect D-dimer values and combined FDP values at the same time.

ABSTRACT

<b>Objective</b> : Although an endoleak is the most common complication after endovascular abdominal aortic aneurysm repair (EVAR), the proper and noninvasive method for the detection of endoleaks is not established. The purpose of this study is to investigate whether plasma levels of D-dimer and fibrin degradation product (FDP) could be predictors of endoleaks after EVAR. <b>Methods</b> : Between June 2011 and January 2014, 65 consecutive patients underwent EVAR at our institution. We evaluated 55 patients excluding 10 patients pre-existing conditions such as aortic dissection, arterial or venous thrombosis, conversion to open surgery, and difficulties in making outpatient visits. Enhanced computed tomography (CT) examination was performed during 12 months after EVAR. Persistent endoleaks and maximum aneurysmal diameter were evaluated at each follow-up time. Patients were divided into groups according to CT findings at 12 months after EVAR. There were 26 patients with endoleaks vs. 29 non-endoleak patients, 34 with unchanged aneurysm findings vs. 21 with shrinkage. No patient showed aneurysmal enlargement. Plasma levels of D-dimer, FDP, counts of platelet, prothrombin time (PT), and activated partial thromboplastin time (APTT) were also measured at the time of CT examinations. <b>Results</b> : There was no operative death and no major complication. Endoleaks in all patients were identified as type II. None of them required re-intervention. In the endoleak group, plasma levels of D-dimer and FDP were significantly higher than in the non-endoleak group in each postoperative period. In addition, postoperative counts of platelet were significantly lower in the endoleak group. PT and APTT test results showed no significant difference in the two groups. In the unchanged aneurysm group, postoperative D-dimer and FDP tended to be higher compared with the shrinkage group. Postoperative counts of platelet also tended to be lower in the unchanged group. There were no differences in PT and APTT test results. <b>Conclusion</b> : Plasma levels of D-dimer and FDP are potentially useful predictors of endoleaks after EVAR.

Introducción: la caries dental es considerada un problema de salud pública, para su tratamiento se han propuesto diversos biomateriales, entre ellos el uso de la mezcla de fluoruro+nitrato de plata, llamada Fluoruro Diamino de Plata (FDP) con propiedades cariostáticas, remineralizantes y bactericidas utilizado en dientes primarios. Son pocos los estudios que valoran la respuesta del tejido dentario luego de aplicarlo, es por ello que el objetivo de este estudio fue determinar el efecto del FDP en caries inducidas en ratas Wistar. Métodos: se hizo inducción de caries dental en molares de ratas Wistar, inoculándolos con S. mutans CVCM656, manteniendo dieta rica en sacarosa durante 12 semanas. Diagnosticadas las caries y distribuidas en grupos control (C1,C2) y experimental (E1,E2), se removió dentina reblandecida de los molares y se topificaron con FDP al 3,8% solo los molares de los grupos experimentales. Finalmente se hizo valoración clínica y toma de muestra para estudio histológico a las 7 y 13 semanas posteriores al tratamiento. Resultados: los grupos control registraron aumento del número de lesiones cariosas y progresión de su severidad, mientras que los grupos experimentales no revelaron cambios en estos parámetros, mostrando superficies dentinarias duras y oscuras, el reporte histológico determinó aumento en el espesor de predentina únicamente en los molares de los grupos experimentales. Conclusiones: el protocolo de inducción de caries en ratas Wistar fue exitoso; asimismo, el tratamiento con FDP inactivó los procesos cariosos sin que progresaran en severidad ni aumentara el número de caries, siendo estos resultados extrapolables a los humanos.


Introduction: dental caries is considered a public health problem. Various biomaterials have been suggested for treating it, including fluride+silver nitrate mix, known as silver diamine fluoride (SDF) which contains cariostatic, remineralizing, and bactericide properties and is used on primary teeth. Only a few studies have assessed the response of dental tissue when this substance is applied; the objective of this study was therefore to determine the effect of SDF on caries induced in Wistar rats. Methods: dental caries was induced in Wistar rats’ molars by inoculating them with S. mutans CVCM656 and feeding them a sucrose-rich diet for 12 weeks. Once caries were diagnosed and sorted out into control groups (C1, C2) and experimental groups (E1, E2), softened dentine was removed from molars; only the molars from experimental groups received a topical application of 3.8% SDF. Finally, clinical assessment was performed and samples were collected for histological study 7 and 13 weeks after treatment. Results: the control groups experienced an increased number of carious lesions and progression in severity, while the same parameters in the experimental groups remained unchanged, showing hard dark dentine surfaces. The histological report showed increased predentine thickness in molars from the experimental groups only. Conclusions: the protocol of caries induction in Wistar rats was successful. Treatment with SDF deactivated the carious processes, arresting not only severity progression but also the increase in number of caries; these findings may be extrapolated to humans.

ABSTRACT

PURPOSE: The purpose of this study was to compare the marginal fit of three-unit zirconia fixed dental prostheses (FDPs) fabricated using CAD/CAM and MAD/MAM system. MATERIALS AND METHODS: Dentiform maxillary central and lateral incisor were prepared for 3-unit FDP and fixed in yellow stone. This model was duplicated to epoxy resin die. On the resin die, fifteen 3-unit FDPs were fabricated. Metal-ceramic group was three-unit metal-ceramic FDPs, Everest(R) group was zirconia three-unit FDPs fabricated using the Everest(R) system (Kavo Dental GmbH, Biberach, Germany) and Rainbow(TM) group was zirconia three-unit FDPs fabricated using the Rainbow(TM) system (Dentium Co. Inc., Seoul, South Korea). They were cemented to resin dies with adhesive resin cement. After removing pontics, each retainers were separated and observed under measuring machine (Presize 440C) and analyzed through one-way ANOVA and Duncan test (alpha= .05). RESULTS: Mean values and standard deviations of marginal gap dimensions in each group for three-unit FDPs were 78.5 +/- 11.05 microm for the metal-ceramic group, 59.30 +/- 11.63 microm for the Everest(R) group and 70.34 +/- 13.98 microm for the Rainbow(TM) group. CONCLUSION: 1. The Everest(R) group in comparison with metal-ceramic group showed better marginal fit, which had significant differences (P.05). 3. The mean marginal gap values between Rainbow(TM) group and metal-ceramic group did not showed significant differences (P>.05). 4. The mean marginal gaps of each group were within clinically acceptable range (120 microm).

ABSTRACT

PURPOSE: To find applicable parameters of the frequency doubling technology perimeter (FDP), which is known to be useful in detecting early visual field loss. METHODS: The subjects were 65 eyes of normal individuals, 58 eyes of glaucoma suspects, and 37 eyes of glaucoma patients. FDP (MD, PSD), SAP (MD, PSD, CPSD), and 5 parameters of OCT were analyzed. The receiver operating characteristic (ROC) curve, sensitivity, and specificity of each parameter were evaluated, and the comparison of FDP parameters by Pearson's chi-square was made between the glaucoma suspect group and the normal group. The same comparison was made among the normal+glaucoma suspect and glaucoma group. RESULTS: In discriminating between the normal and glaucoma suspect groups, the FDP-PSD revealed an AUC of 0.745 (cutoff: 3.17dB), which was significantly larger than that of SAP-PSD with an AUC of 0.660 (cutoff: 1.73dB). In comparing FDP, PSD by Anderson criteria was most reliable with a hit ratio of 0.732. On the other hand, in discriminating among the normal+glaucoma suspect group and glaucoma group, the SAP-PSD showed an AUC 0.971 (cutoff: 2.92dB), much larger than the FDP-PSD with an AUC of 0.863 (cutoff: 3.78dB). In addition, in comparing the parameters of FDP by Pearson chi-square, PSD 1% was the most reliable with a hit ratio of 0.813. CONCLUSIONS: FDP is a valuable screening tool in detecting early visual field loss. PSD by Anderson criteria can be a reliable parameter for the diagnosis of pre-perimetric glaucoma, and PSD 1% can be used in the discrimination of perimetric glaucoma.

Objective To investigate the protective role of FDP to STZ induced islest apoptosis and the potential mechanisms.Methods The pancreases of the rats were treated to collect islets cells.The cells were incubated with STZ with/or FDP.Cell morphology,insulin secretion,HO-1 activity,CO content,SOD activity,GSH-px activity,iNOS activity were examined.No conetent and apoptotic percentage was detected.Results HO-1 activity and CO content of the normal control group were low.STZ induced a significant decrease of cell activity and insulin release,flow cytometry analysis showed that apoptotic percentage of islet cells remarkably increased following the addition of STZ,FDP obviously improved the islets cellular activity damaged by STZ,basic amount of insulin secretion and stimulated by high glucose were improved(P

ABSTRACT

BACKGROUND: The DR-70(TM) immunoassay is a newly developed cancer diagnostic test which quantifies the serum fibrin degradation products (FDP), produced during fibrinolysis, by antibody reaction. The purpose of this study was to evaluate the potential of DR-70(TM) immunoassay in screening malignant tumor. METHODS: Sample subjects were 4,169 adults, both male and female, who visited the health promotion center of a general hospital from March 2004 to April 2005 and underwent the DR-70(TM) immunoassay test and other tests for cancer diagnosis. The patient group was defined as 42 adults out of the sample subjects who were newly diagnosed with cancer during the same time period when the DR-70(TM) immunoassay test was performed. Final confirmation of a malignant tumor was made by pathological analysis. RESULTS: The mean DR-70(TM) level was 0.83+/-0.65 microgram/ml (range: 0.00 (0.0001)~7.42 microgram/ml) in the control group (n=4,127) as opposed to 2.70+/-2.33 microgram/ml (range: 0.12 ~ 9.30 microgram/ml) in the cancer group (n=42), and statistical significance was established (p<0.0001, Student t-test). When categorized by the type of malignant tumor, all cancer patients with the exception of the subgroups of colon and rectal cancer showed significantly higher mean DR-70(TM) levels compared with the control group (p<0.0001, Kruscal-Wallis test). The receiver operating characteristic (ROC) curve analysis revealed < or = 1.091 microgram/ml as the best cut-off value. Using this cut-off value, the DR-70(TM) immunoassay produced a sensitivity of 71.4%, a specificity of 70.1%, a positive predictability of 69.4%, and a negative predictability of 69.2% (1). CONCLUSION: A significant increase in the mean DR-70(TM) value was observed in the cancer group (thyroidal, gastric, breast, hepatic and ovarian) compared with the control group. In particular, the specificity and sensitivity of the DR-70(TM) immunoassay was relatively high in the subgroups of breast, gastric, and thyroidal cancer patients. There is need for further studies on a large number of malignant tumor patients to see how the DR-70(TM) level might be changed according to the differentiation grade and postoperative prognosis of the malignant tumor.

ABSTRACT

Summary: In this study we tried to investigate the effect of fructose-1,6-diphosphate and HTK solution on protecting primary cardiac muscle cells of rat with cold preservation. The primary cardiac muscle cells of rat were cultured in vitro with four preservation solutions respectively: 0.9 % sodium chloride solution (group A), FDP (group B), HTK solution (group C) and a mixture of FDP and HTK solution (group D). The cells were preserved for 6, 8 and 10 h at 0-4 ℃. The values of AST and LDH-L and the Na+-K+ ATPase activity in cardiac muscle cells were detected, and the survival rate of cardiac muscle cells was detected with trypan blue staining. The values of AST and LDH-L in group C and group D were remarkable lower those in group A and group B (P<0.001), while the Na+-K+ ATPase activity and the survival rate of cells in group C and group D were much higher than those in group A and group B (P<0.001). The values of AST and LDH-L after 6 hours in group D decreased much more than those in group C (P<0.01), while the Na+-K+ ATPase activity and the survival rate of cells in group D improved more than those in group C (P<0.01). Both of the HTK solution and the mixture of HTK and FDP solution have an evident effect on protecting the primary cardiac muscle cells of rat in vitro with cold preservation, Compared with the HTK solution, the mixture solution has a better short-term protective effect.

ABSTRACT

PURPOSE: To study the unaffected fellow eye in patients with normal tension glaucoma and unilateral visual field defect in conventional white-on-white automated perimetry, using scanning laser polarimetry and frequency-doubling perimetry. METHODS: Thirty patients with a unilateral glaucomatous visual field defect detected on conventional perimetry (Humphrey field analyzer) were selected. The contralateral eyes with normal conventional perimetry results were studied using scanning laser polarimetry and frequency-doubling perimetry. Twenty eyes of 20 age- (+/-5 years), sex-, refractive error- (+/-2 D) matched persons without glaucoma were selected as a control group. RESULTS: Of the 30 contralateral eyes, 18 eyes (60%) showed a defect of the nerve fiber layer when evaluated with SLP. In the control group, 2 eyes (8.3%) showed a defect of the nerve fiber layer. Of the 20 contralateral eyes, 14 eyes showed abnormal FDP results. In the 17 control group, 2 eyes (2%) showed abnormal FDP results. CONCLUSIONS: These findings suggest that what appeared to be unilateral visual field defects by conventional white-on-white automated perimetry may in fact underestimate structural as well as functional glaucomatous damage in the contralateral eyes. These results may be important for early detection of glaucoma and future treatment.

ABSTRACT

OBJECTIVE: The purpose of this study was to evaluate the incidence of inability to flex proximal interphalangeal (PIP) and distal interphalangeal (DIP) joint in normal Koreans. The opposition palm ratio and thumb index ratio was also of interest. METHOD: Randomly selected eighty nine normal Korean adults of 48 men and 41 women, with ages 20 to 79 years. The finger flexion was measured using the standard flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) screening tests. Thumb index ratio and opposition palm ratio was also assessed additionally. RESULTS: In the sample population, 2 subjects (2.2%) were unable to bilaterally flex their fifth PIP joints independently. Four subjects (4.4%) were unable to flex one or both of their fourth or fifth DIP joints. These results show far less incidence of FDS dysfunction comparing with 52% of FDS dysfunction rate in Americans. The opposition palm ratio in men were 63.6% and in women 69.9%, indicating lower ratio in men. The thumb index ratio was not different statistically between the men and women. CONCLUSION: These data suggest that the different incidences of FDS and FDP dysfunction should be considered in interpretation. The opposition palm ratio and thumb index ratio might be useful in the hand rehabilitation.

ABSTRACT

We evaluated the sensitivity and specificity of Frequency doubling perimetry[FDP]with fourty-nine normal subjects and twenty-six patients of early glaucomatous field change. We regarded Humphrey field analyzer[HFA]30-2 GHT as gold standard. 'True positive 'was regarded as 'outside normal limit 'or P-value of MD or PSD below 0.05 at HFA, 'abnormal 'at FDP P-value of MD or PSD below 0.05. MD of early glaucoma was -5.4 decibel at HFA and -6.61 decibel at FDP, Sensitivity of early glaucoma was 96.1%with FDP and 84.6%with HFA;specificity, 89.7%with FDP and 83.6%with HFA. We could confirm FDP threshold test showed good sensitivity in detecting early glaucoma. Especially, use of PSD increased efficiency of the test.

ABSTRACT

In general, FDP and D-dimer values have a correlation in clinical conditions associated with disseminated intravascular coagulation(DIC) or coagulation activation. However, there are some patients with discordant results who demonstrate elevated FDP and negative D-dimer results by latex agglutination assays. The incidence and possible reasons for the discordance between FDP and D-dimer results were investigated through simultaneous measurements (n = 763) from clinical patients with suspected DIC or coagulation activation. 24.8% (189/763) of samples with elevated FDP were negative for D-dimer assays by the latex agglutination method. Further detailed analysis on randomly-selected discordant samples (n = 41) revealed that the most common reason for the discordance was the lower sensitivity of the semiquantitative latex agglutination method for D-dimer, compared with quantitative enzyme or other latex immunoassay. The other contributing factors to the discordance were accelerated fibrinogenolysis without secondary fibrinolysis, elevated soluble fibrin monomer and rheumatoid factor.

ABSTRACT

The study was designed to observe the effects of fructose- 1 ,6 - diphosphate (FDP) on the stressed patients who were supported with total parenteral nutrition (TPN) after gastrectomy.Twelve cases of patients with gastric cancer were randomly divided into two groups.The control group was supported by TPN,and the FDP group received TPN plus FDP.The resulshowed thatin FDP group the serum levels of cortisol and glucogon declined and the urine 3- methyl- histidine excretin decreased with a better cumulative nutrogen balance when compared with control group.It sug- gested that FDP may lessen the protein catabolism and improve the results of nu- tritional support in stressed patients after trauma or surgical operation.

ABSTRACT

Thrombolysis using urokinase solution is one of the effective methods in treatment of intracerebral hematoma. The present study was undertaken in order to 1) determine the most effective concentration of urokinase solution, 2) determine the most suitable time interval of irrigation of urokinase solution through the measurement of hemoglobin and FDP(Fibrin / Fibrinogen Degradation Product) of drained solution, 3) estimate the size of unresolved hematoma without taking brain CT. The results are summarized as follows : 1) The most effective and economic concentration of urokinase solution was 1000 u/ml. 2) The most preferable time interval of irrigation of urokinase solution was about one hour. 3) It was possible to estimate the size of unresolving hematoma by means of measurement of hemoglobin and hematocrit of patient, hemoglobin and volume of the thrombolysed solution, so that it was unnecessary to take brain CT for measurement of remaining hematoma.
